These SMS Terms and Conditions apply to transactional text messages sent through the Moyava Clinical text messaging program, a service operated by Moyava Health LLC.
When you opt in, Moyava Clinical may send you non-marketing text notifications. Moyava Clinical is the sender identified in these messages. Messages may notify you that clinician-selected educational materials or related materials are available through a secure link.
The ordinary text message is designed not to include the patient's name, diagnosis, condition, medication, or other clinical details. Clinical or personalized material, when included, is accessed through the secure link.
You may opt in on the Moyava Clinical consent form by actively selecting two separate boxes, each of which starts unchecked: one consenting to receive text messages at the mobile number provided, and one accepting these SMS Terms and acknowledging the Privacy Policy. By opting in, you confirm that you are the subscriber or customary user of that number and are authorized to consent for that number. Moyava Health LLC receives and controls that consent.
Consent is optional and is not a condition of receiving healthcare services, education, or other materials. You may continue without SMS and use an available QR code, printed copy, or copied secure link.
Message frequency varies based on clinic visits and requests to deliver clinician-approved materials. The program is transactional and non-marketing.
Message and data rates may apply. Moyava Health LLC and participating clinics do not charge a separate fee for the text message, but your wireless carrier's rates may apply.
Reply STOP to the sending number to opt out. You will receive one final confirmation message, and no further Moyava Clinical messages will be sent from that number unless you opt in again.

Reply HELP for help information. You may also email support@moyavahealth.com for program support. Do not send medical questions or emergency requests by text.
Delivery is subject to wireless-network availability and is not guaranteed. Carriers are not liable for delayed or undelivered messages. Secure links may expire or be revoked.
This text program is not an emergency service and is not monitored for medical replies. Contact the treating clinic for medical questions. Call 911 for an emergency.
Our collection and use of information is described in the Privacy Policy. Text messaging originator opt-in data and consent will not be sold, rented, transferred, or shared with any third parties or affiliates, and no mobile information will be shared with third parties or affiliates for marketing or promotional purposes.
Message delivery may depend on your wireless carrier and device. Supported carriers and features may change without notice.
We may update these SMS Terms. We will post the revised version with a new effective date and provide any notice required by law.
